Request to be Guardian of a Mentally Retarded Adult Child, Brother or Sister PS-11
Parents and brothers or sisters who care for a mentally retarded relative in their home may use this form to ask the Court to be appointed guardian so they may continue to care for their relative. You must provide a medical report, describing your relative's mental retardation and ability to care for himself or herself, to the Court in addition to this form.
